• Below is a list of insurance companies with which Pinnacle Family Practice participates.  Please note that it is the responsibility of each individually insured patient to know what is and is not a covered benefit under their insurance plan. Every single plan is unique.   If you are seen for a non-covered benefit, it is the patient’s responsibility for all fees related to that visit.  If you are unsure if your visit, procedure, vaccine, or lab is covered, it is the individual’s responsibility to contact their insurance plan carrier to determine coverage. 
• Due to current federal and insurance regulations, all co-payments, co-insurance, and deductibles are collected at the time of service. We accept cash, checks, debit cards, and Visa/Mastercard.
• If an individual has an HMO plan, that individual is required to designate their primary care physcian (PCP) as Dr. Saksena AND confirm that it is effective prior to the appointment.  If not fully effective, then the individual will be responsible for all fees for that visit.
• It is the individual’s responsibility to know what their annual deductible is and whether or not that deductible has been met.
